Resumen : La articulación de la rodilla es la que causa mayor número de patologías y cirugías dentro de la especialidad. La cirugía del ligamento cruzado anterior(LCA) es la cuarta mayor cirugía en el mundo. La incidencia de rupturas del LCA en la población general representa entre el 0,01% y0,05%. El LCA se inserta en la cara anteromedial y posterolateral en relación a su inserción en la espina tibial. El LCA tiene su origen en la cara medial del condigo femoral lateral en dirección medial, anterior y caudal. LCA se divide en 2 haces: anteromedial y posterolateral en clara relación con su inserción en la espina tibial. Las roturas del LCP son mucho menos frecuentes que el LCA por la propia biomecánica de los movimientos y constituye una estructura menos estudiada. El LCP se inserta en la cara lateral del condilo femoral medial. Discurre tridimensionalmente hacia lateral, posterior y caudal hasta alcanzar los haces anterolateral y posteromedial la cresta intercondílea de la tibia, extendiéndose unos milímetros por la superficie tibial posterior. Su objetivo primario es limitar la translación anterior de la tibia con respecto al fémur así como limitar la rotación externa y las rotaciones en varo y valgo de manera secundaria. Biomécanicamente la actitud y la flexión del LCP es difícil de estudiar tanto en vivo como en cadáver. Existen dos LMF que unen el cuerno posterior del Menisco externo (ME) con la cara lateral del condilo femoral medial. Existen estudios que hablan de variantes con inserción parcial en el LCP. El ligamento meniscofemoral anterior (LMFA) es conocido como ligamento de Humphrey y el ligamento menisco femoral posterior (LMFP) es conocido como ligamento de Wrisberg. El ligamento de Humphrey esmás pequeño y delgado y está en íntima relación con el LCP. El ligamento de Wrisberg: es un "punto" de baja señal en dirección posterosuperior al LCP en imagenes sagitales. Histologicamente las raíces meniscales estás formados por: fibras meniscales, firbrocartílago calcificado, fibrocartílago descalcificado y hueso. Estudios es cadáver indican que el LMFA está presente en el 50% de los casos mientras que LMFP en el 76%. Un estudio radiológico reciente de 138 rodillas habla de la presencia de algún LMF en el 83% de los casos. En cuanto a la funcionalidad diversos autores se refieren a los LMF como una estructura vestigial sin ningún tipo de funcionalidad. Otros autores afirmar que los LMF protegen el compartimento lateral tras la ruptura avulsión de la raíz del ME. Radiológicamente se han descrito pseudo ruptura definidas como: cambio alteración de la densidad entre el menisco y los LMF. Se han descrito 2 patrones: 13% rupturas verticales, 87% rupturas de orientación anterosuperior y posteroinferior. Se ha identificado la presencia de ambos ligamentos en el 3% de la resonancias, presentando aisladamente el LMFA en el 4% y el LMFP en el 30% . Respecto a la clasificación destacan 3 autores: Nagasaki et al. (2006) habla de 5 tipos en función de la inserción en el cuerno posterior del ME, usando modelos de prótesis total de rodilla: próxima, central, distal, toda la superficie posterior, superficie temporal del menisco lateral. Cho et al.(1999) categoriza las variaciones del LMF en 3 grupos: inserción en el condilo femoral medial, inserción entre el condilo femoral medial y tercio próximal del LCP, inserción entre el condilo femoral medial y el tercio femoral distal del LCP. Han et al. (2011): Ia: ausencia de LMFP; Ib: ausencia de LMFP pero presencia de banda oblicua en el LCP; IIa: presencia de LMFP con inserción femoral alta; IIb: presencia de LMFP con inserción femoral baja; IIc: mezclado con fibras del LCP; IIId: presencia de LMFP y presencia de banda oblicua en el LCP . Vamos a centrar nuestro estudio en: 1.-ligamento cruzado posterior (LCP) 2.- relación del ligamento cruzado posterior- anterior 3.relación escotadura intercondílea- ligamentos cruzados 4.— anatomía de los ligamentos meniscofemorales (LMF) y relación con los ligamentos cruzados 5.- relación estructural de la cápsula articular con respecto a los elementos posteriores de la rodilla. En nuestro estudio consideramos elementos posteriores de la articulación de la rodilla: cápsula posterior, ligamento cruzado posterior, ligamentos menisco femorales.

The posterior cruciate ligament (PCL) is intraarticular, although extrasynovial, wide, and varies in each individual. It follows an oblique path upwards, forwards and inwards, with a curved configuration to save the posterior border of the proximal end of the tibia. It is flatter and thinner than the anterior cruciate ligament (ACL). The tibial insertion, unlike the ACL, is located in its posterior cortex and reaches one centimeter distal to the joint interline and slightly lateral; It is smaller than the femoral. Like the ACL, the PCL is made up of a set of fibers that constitute two fascicles, the antero- lateral (AL) and the postero-medial (PM). The AL bundle tightens during knee flexion, while the PM does in extension, but the two PCL bundles have a synergistic relationship during joint mobility. For their part, the menisco-femoral ligaments (MFL) originate in the posterior horn of the external meniscus and insert into the internal femoral condyle in front (Humphrey's ligament) and behind (Wrisberg's ligament) of the PCL. Its dimensions and presence are variable. Its function is to prevent excessive extrusion of the meniscus under axial stresses in the case of tears of the posterior horn of the external meniscus. The PCL is a constant anatomical structure, with the meniscus- femoral ligaments being accessory structures that stabilize its anchorage. In the present work we will analyze the dimensions of the PCL, ACL, presence of the menisco- femoral ligaments in human knees and the correlation with the dimensions of the knee skeleton. It has been argued that the menisco-femoral ligaments disappear with age, due to microtraumatisms that end up breaking them; For this, we have analyzed the presence of the meniscus-femoral ligaments, in MRI, according to sex and age, as well as the dimensions of the ACL and the PCL. Material and Methodology Anatomical study on 30 specimens of human knees, dissected with the same protocol: dissection of the skin and subcutaneous cellular tissue. The capsule was opened with a parapatellar incision to observe the existence and visualize the ACL. One knee presented a stump as an ACL, so it was discarded; 16 were on the right side and 13 on the left. Once the presence of the ACL was confirmed, the posterior aspect was dissected, visualizing the posterior cruciate ligament, cleaning its origin, trajectory and insertion, dissecting, when present, the menisco-femoral ligaments and the posterior horn of the external meniscus. We measure the length and width of the cruciate ligaments with a vernier caliper, anteriorly at 90° and posteriorly at full extension. We measured the maximum antero-posterior diameter of the femoral condyles and the proximal end of the tibia; maximum transverse diameter of the femoral condyles and of the proximal end of the tibia, as well as the dimensions of the femoral 21 notch, height, width and depth. In addition, we measure and analyze the presence of the meniscus ligaments – femoral, anterior and posterior. ACL length was measured with different degrees of knee flexion. To do this, we placed the knee on a monopod support and the length of the ACL was measured at different degrees of flexion with a goniometer. Each length measurement was made three times, and the mean of the three measurements was noted. The width was measured, in each of the ligaments, three times in the proximal area and three times in the distal area, noting the average of the three measurements. Once the cruciate, ACL and PCL ligaments were measured, they were sectioned in their most proximal portion, after which we extracted the posterior horn of the external meniscus and the two meniscus-femoral ligaments, measuring their length, noting their shape and presence. A histological study was performed, Masson's trichrome staining, when possible. We analyzed 120 magnetic resonance imaging (MRI), 51 corresponded to women and 69 to men, in people who did not suffer from knee pathology, studied in the Radiodiagnosis service of our hospital. The female knees were 25 on the right side and 26 on the left side, the mean age was 33 (SD: 14; range: 12 – 63) years, and the male knees were 40 on the right side and 29 on the left, the mean age was 37. (SD: 13; range: 9 – 65) years. We included in the study three cases of chondral injury to the medial femoral condyle, three cases of traumatic patella dislocation, and three cases of medial collateral ligament rupture, all treated with conservative methods at least three years earlier.
